36-Item Short Form Survey

What is SF-36

The 36-Item Short Form Survey (SF-36) is a set of generic, coherent, and easily administered quality-of-life measures. It taps eight health concepts: physical functioning, bodily pain, role limitations due to physical health problems, role limitations due to personal or emotional problems, emotional well-being, social functioning, energy/fatigue, and general health perceptions. It also includes a single item that provides an indication of perceived change in health.

Notes

Since the questionnaire relies on client self-report, all responses should be verified by the clinician, and a definitive diagnosis is made on clinical grounds taking into account how well the client understood the questionnaire, as well as other relevant information from the client.

Question 2 covers change in health status over the past year; this is not counted in scoring the eight dimensions, but is used to estimate change in health from a cross-sectional administration of the SF-36.

The survey may be used with disease specific measures as an outcome measure in clinical practice and research.
